Its Month 2 of 2020,
That was kind of cool when we had 02/02/2020, an interesting day that won't come around too often.
Lets get straight into things, I am going to get into a subject that we will cover over the next few months, first lets cover a couple of things that are quite topical - well they are in my world. Remember to send in any questions you may have as I am happy to answer them for you.

Gut health in the 2020's Part 1.
Over the next few months I will be talking about different aspects of our GUT health, and digging into some of the latest info about ways to improve your gut, and as a result improve a whole myriad of concerns.
In a very basic assessment there are a few things our GUT is there to do:
1-Its where we put food in and hope that our GUT will be able to process/digest it and get some fuel into the blood from nutrients broken down and assimilated.
2-Its also where we eliminate waste left over from our dietary intake.
3-And the third thing we need to understand - Its where most of our immune response is initiated - its said about 70% of our immune system and activity takes place or is initiated in the gut.
So for these reasons stated above - the health of our gut is critical to our overall health, in many ways.
Many of you will be familiar with the famous quote - "death starts in the colon" - some suggest this phrase came from Hippocrates but it may well have come from Dr Bernard Jensen, the well respected doctor who taught iridology and was known for his powerful, effective body cleansing that he used and talked about long before most people had any understanding of the importance the gut.
In my 20 years of naturopathic practice the importance of the gut in all areas of health has certainly been re-enforced time and time again. What has changed is the science and also the way I approach this important area.
When I was first in practice we would use protocols based around the "weed-seed-feed" narrative, this would mean first to come in with powerful anti-microbial herbs to hammer any bad bugs whether they were fungal, viral, bacterial or parasitic. The follow up to this "weeding" would be then to seed or prepare the ground with soothing, gut repairing nutrients that would help deal to any weakness in the gut, and then give a general all round boost of all the necessary probiotics that would proliferate and help to maintain the "healthy gut".
Fast forward to 2020 and we have seen massive growth in our understanding of probiotic's, prebiotic's and other aspects of gut health. There is now significant science showing that quite specific bacteria have quite specific roles and benefits and that many species don't in fact colonise but are transitory.
For me I have also come to the place where coming in hard with anti-microbial herbs may still be appropriate in some cases, however I prefer to come from a slightly different angle to get a dysfunctional gut back to normal operation, the approach is very different, but also very powerful, as when we restore functionality, our body has this amazing ability to move into true self healing.
So what does this look like?
Well part of this comes from what is now being referred to as the GEMM protocol and this is exciting stuff.
G.E.M.M. - Gut Ecology and Metabolic Modulation. Once again, our gut is central to our wellbeing, and the metabolic modulation relates to all of the issues modern diets and lifestyle have caused, heart disease is still the number one killer - even though the drug companies have billions and billions of $$$ worth of drugs prescribed every year to help with hypertension, high cholesterol and atherosclerosis and strokes - so obviously something has been missed, and its working with your body by getting your system back to normal, from the inside out.
Before I get to GEMM, I want to take a step back and get you to be honest about one fundamental thing, and that is your poop. Yes, we know we have to poop, because as it's been said in the past - "if you don't poop, you die". This is so important as our modern societies, including NZ, rate very high on the colon cancer list, as well as other mostly avoidable illnesses. Colon or colorectal cancer is in the top 5 of cancer statistics for NZ, Australia and the USA.
Consider also the chronic constipation experienced by huge numbers of people, conditions such as Enlarged Hemorrhoids or Irritable bowel syndrome, and then there is Diverticulitis and Diverticulosis occurring in a big chunk of the 60-80 year olds, we are definitely missing something here.
So it's kind of like a bit of reverse engineering, what we end up eliminating can tell us things about what is (or isn't) going in at the beginning.
A tool that is often used is the Bristol Stool Chart. You may have seen this in your travels, but what does it mean? The chart is the creation of Researchers at the Bristol Royal Infirmary - a hospital in Bristol, England - and is a visual guide for stools. It is called the Bristol Stool Form Scale, or BSF scale for short.
If we first understand what our poop is telling us, then we can start to make corrections and improve things.
Contrary to what your Doctor may tell you - having a poop once or twice a week is NOT OK! Through our diet and our training or conditioning most of us moved out of the habit we had as babies of pooping whenever it felt right. As adults with certain commitments and time constraints due to our job or work place we tend to hold onto our poop more often and this is not ideal, but I understand its just how it is.
Let's run over what you might see.
This is a summary, I put it here for those who don't want to read through all the info :)
The following definitions simplify the chart:
- Types 1, 2 and 3 = hard or impacted stools.
- Type 4 and 5 = normal or optimal.
- Type 6 = loose stool, subnormal, or suboptimal
- Type 7 = diarrhea.
For those who like a little additional understanding - please read on:
Type 1: Separate hard lumps, like nuts
This is typical for acute dysbiosis - or microbial imbalance - and commonly seen post-antibiotic treatments. These stools lack a normal shape or form, and this is largely due to the lack of bacteria. Bacteria in the stool are necessary to retain water. Single cell organisms, such as bacteria, contain mostly water. The lumps are hard and even abrasive, ranging from 1 to 2 cm diameter, because the lumps are hard and scratchy they may in fact be painful to pass. There is even a likelihood of bleeding due to extreme mechanical forces(forcing). Flatulence isn‘t likely, because with no/low bacteria, fermentation of fibre won't be happening.
Type 2: Sausage-like but lumpy
This is a combination of a little bit of fibre from the diet and maybe some bacteria along with a Type 1 stool.
The size of the stool is a problem. (could be a diameter of 3 to 4 cm) The anal canal‘s aperture is only around 3.5 cm so you can see the probability of extreme straining during elimination is high. To get to this size, the stools have probably been in the colon for at least several weeks instead of the normal 72 hours and this of course will have other ramifications aside from the pain and potential damage when attempting to eliminate. Withholding or delaying of pooping, Hemorrhoidal disease, anal fissures, and a history of chronic constipation are the most likely causes. It's likely there will be some flatulence with the length of time the stool has been "incubating". With continuous pressure of large stools on the intestinal walls is possible that these people will suffer from irritable bowel syndrome. The possibility of obstruction of the small intestine is high, because the large intestine is filled to capacity with stools. These are the types of people who will have a real problem if you simply add supplemental fibre into their routine, because the expanded fibre has no place to go. This could even cause hernia, obstruction, or perforation of the small and large intestine, so don't do it unless under supervision.
» Type 3: Like a sausage but with cracks in the surface
This next "sausage" has the same characteristics of Type 2 stools, with one main difference - a faster transit time, between one and two weeks. The typical term used here would be latent constipation. The diameter is 2 to 3.5 cm. As the stool isn't as enlarged as Type 2, this suggests that the defecations are regular, however straining will be normal and required. Irritable bowel syndrome is highly likely. Flatulence is likely due to dysbiosis, but probably minor. All of the adverse effects typical for Type 2 stools will also be risks for these people, especially the rapid deterioration of hemorrhoidal disease.
» Type 4: Like a sausage or snake, smooth and soft
This form is normal for someone defecating once daily. The diameter is 1 to 2 cm (0.4–0.8”). A larger diameter will indicate a longer transit time or a large amount of dietary fiber in the diet.
» Type 5: Soft blobs with clear-cut edges
Here we go, what is perhaps the most ideal. This is what I would consider typical for a person who has stools twice or three times daily, after major meals. Food goes in, and waste moves out, this is how the natural cycle likes to work. The diameter will be 1 to 1.5 cm.
» Type 6: Fluffy pieces with ragged edges, a mushy stool
This form may not be much fun, it may be difficult to control the urge, especially when you don‘t have immediate access to a bathroom. Second, it's potentially a messy affair to manage with toilet paper alone, this is where a flexible shower head or even bidet would be great. Consider this as borderline normal.
These kind of stools may suggest a slightly hyperactive colon, and hyperactivity could be caused by a number of things - excess dietary potassium, or sudden dehydration or spike in blood pressure related to stress.
» Type 7: Watery, no solid pieces
This, of course, is diarrhea or "the runs". Most of us have experienced this at some time and hopefully it is something you and your body overcame without too many problems. The weird thing is that there is what we call - paradoxical diarrhea. It‘s typical for people with impacted stools—such as type 1 stools. During paradoxical diarrhea the liquid contents of the small intestine (up to 1.5–2 liters daily) have no place to go but down, because the large intestine is stuffed with impacted stools throughout its entire length. Some water gets absorbed, the rest squeezes past the impacted stool and accumulates in the rectum. The reason this type of diarrhea is called paradoxical is because being severely constipated and experiencing diarrhea all at once, is of course, a paradoxical situation. Unfortunately, it‘s all too common. The swinging from constipation to diarrhea is possibly a sign of IBS.
Here again is a summary of the definitions:
Types 1, 2 and 3 = hard or impacted stools.
Type 4 and 5 = normal or optimal.
Type 6 = loose stool, subnormal, or suboptimal.
Type 7 = diarrhea.
So what can I do with this information?
Understanding your poop is a simple first step in determining if it's appropriate to change something in your life, and if you are in type 1-3 definition - impacted stool - I want you to put your laxatives and fibre supplements aside for now and consider doing this one thing for me, now don't take this as the only thing to do, but it's a start.
I want you to consider getting a good daily dose of a high quality Vitamin C - our old favourite Daily C will do the job to get things started. All you do is simply take it first thing in the morning on an empty stomach, start with half a teaspoon, and if you need/want to take a little more increase it slowly up to a full teaspoon if required.
